Licensed Vocational Nurse (LVN)
This is a Field Position
Coverage areas include Greater Los Angeles
Hourly Rate $35-40 + mileage

POSITION SUMMARY:
The Clinical Care Coordinator is a nurse who engages in
community outreach to counsel, explain and educate patients and families about
hospice and palliative care services, reviews consents and other documents and
assists with election of hospice benefit. The Clinical Care Coordinator
coordinates patient discharge from hospital into hospice or palliative care
programs and performs start of care and initial nursing assessment. The
Clinical Care Coordinator works cooperatively in coordination with the hospice
and/or palliative care teams. The
Clinical Care Coordinator requires desire to work directly with patients and
families to assist them efficiently accessing hospice and/or palliative care
services. Must be able to utilize computers and be
comfortable with electronic medical record. Must be able to meet with patients and
families in person and/or remotely
QUALIFICATIONS:
- Graduate of accredited
school of nursing with current nursing license in California.
- Certification in Hospice
and Palliative Nursing encouraged.
- At least one-year experience as a professional
nurse within the last 3 years
- Excellent verbal and
written communication skills.
- Spanish speaking a plus,
but not required.
- Valid driver’s license, and auto insurance, reliable and well-maintained transportation.
